Most of us associate broken or severely chipped teeth as a common, or at least, accepted event that occurs during childhood. Repair of children's teeth before the adult or secondary teeth is frequently managed with composite resin bonding.

Restorative composites are the best treatment choice for such injuries and also minor injuries that might occur in adulthood.... with adult teeth. Re-establishing normal dental function can be achieved in most cases, along with an acceptable aesthetic or cosmetic result.

Beyond Minor

Adult tooth replacement or repair dentistry needs arising from trauma can become quite complex. As everyone should know, the growth of new teeth doesn't occur more than once... for most of us.

For adults, occupational activities as well as social activities (and everything in between) become the sources of potential trauma. Sports activity, for example, can be career related or simply a social event.

The first photo is our series of dental trauma dentistry shows what can happen when an executive attends a business - social event and demonstrates his talent for swimming. He dove into a swimming pool, but instead of hitting the water first, his upper jaw hit the edge of the swimming pool first. The lower, mandibular arch of anterior teeth have been replaced with a temporary prosthetic bridge. What remains is a partially edentulated upper anterior arch.

The second picture shows the placement of an upper implant supported fixed bridge along with a view of how implants were placed in the lower jaw in preparation for accommodating another prosthetic bridge. Tissue and bone resorption, due to adult gum disease, was a factor in the implant surgeries performed.

The rehabilitation "interests" of this patient exceeded what others may consider as a basic need. This patient has a high visibility career and having teeth that look like natural teeth, complementing his overall facial appearance, is critical.

He simply wanted results that provided the best function possible, on par with what Mother Nature had previously provided, but without the gum disease component.

The third and last photo representing the final outcome of the cosmetic and prosthetic procedures used, show two complete upper and lower arches of teeth that create the degree of cosmetic brilliance the patient wanted.

Vertical dimension was increased to compensate for the loss in height caused by bone and tissue loss that wasn't resolved via bone grafts. Mild variations in texture, sizing and positioning promoted the appearance of natural teeth... as opposed to text book perfect (artificial looking) teeth.

Hitting the steering wheel in a minor fender bender can be a significant event for some patients.

This before treatment photo shows the extent of damage to two of the patient's anterior teeth. Treatment choices included implants and crowns or a custom bridge that could be cemented or implant supported.

An unusual example of how a "direct hit" trauma event can not only break individual tooth structures, but can cause significant damage or change to the jawbone structures and bone-root relationships to anterior teeth.

This photo, taken immediately after the injury readily shows a severely fractured anterior tooth. Closer inspection also reveals changes in tooth position and damage to the upper boney ridge.... which caused additional tooth structures to drop lower, disrupting the patient's incisal bite, occlusion and alignment.

Orthodontic Trauma

This adult patient had numerous orthodontic issues that should have been treated during his developmental years, to take advantage of certain growth milestones that could have created a normal bite and dentition for the patient.

It is not unusual however for patients to delay treatments due to any number of possible reasons.... the ones we all have for not getting treatment at a certain time.

Now an adult, with orthodontically caused tooth breakage, the patient is prepared to implement a variety of procedures that will normalize the bite, reposition opposing teeth relationships, correct sizing relationships and to replace what is beyond minor repair.
